Please print this form, fill it out in its entirety, and sign the final page. It should be turned in to your counselor at the time of your first visit.
Child History
The purpose of this form is to provide the counselor at Solid Ground Counseling some general information about the child or adolescent for whom you seek therapy. This form also asks questions regarding background information in the areas of family structure, health concerns, & client's presenting problem. Please print this form, fill it out in its entirety and sign the final page. It should be turned in to your counselor at the time of your first visit. Please note that this should be done by the child or adolescents legal guardian.
